Academic Integrity Procedures

Purpose
These procedures establish the process for reporting, reviewing, and resolving alleged violations of the Academic Integrity Policy. The procedures are intended to ensure consistent application of the policy, maintain accurate institutional records, and provide students with due process.
Reporting Academic Dishonesty
A. Instructor Responsibility
When an instructor determines that a student has violated the Academic Integrity Policy, the instructor must:
- Address the matter directly with the student, as appropriate.
- Determine and apply any academic penalty authorized under the Academic Integrity Policy and course syllabus.
- Complete and submit the Academic Dishonesty Report Form.
Academic Dishonesty Report Form
The report should include:
- Student name and ID number
- Course and section information
- Date of the incident
- Description of the alleged violation
- Supporting documentation or evidence
- Academic penalty assigned by the instructor
B. Distribution of Reports
Submitted reports are automatically routed to:
- The Dean of Students Office
- The Registrar's Office
The Registrar maintains institutional records of reported academic integrity violations.
First Academic Integrity Violation
Upon receipt of a first Academic Dishonesty Report, the Dean of Students Office will:
- Review the report for completeness.
- Verify whether the student has any prior academic integrity violations on record.
- Send written notice to the student regarding the reported violation.
- Place the student on Deferred Adjudication as outlined in the Academic Integrity Policy.
Deferred Adjudication serves as an institutional warning and provides the student an opportunity to avoid further disciplinary action by complying with university expectations.
Student Right to Appeal
A student may appeal the finding that an academic integrity violation occurred.
Appeals must:
- Be submitted within seven (7) calendar days of the notice date.
- Be submitted using the Academic Integrity Appeal Form.
Academic Integrity Appeal Form
Students may appeal the determination that a violation occurred but may not appeal the academic penalty assigned by the instructor.
Examples of academic penalties that are not subject to appeal through this process include:
- Reduced grade on an assignment
- Grade of zero on an assignment
- Course grade penalties authorized by the policy
Academic Committee Review
Appeals are reviewed by the Academic Committee in accordance with university procedures.
The Academic Committee will:
- Review the appeal and supporting documentation.
- Review information submitted by the instructor.
- Determine whether the reported violation is supported by the evidence.
The Academic Committee may:
- Uphold the finding of academic dishonesty; or
- Overturn the finding of academic dishonesty.
The decision of the Academic Committee is final.
Second Academic Integrity Violation
If a student is reported for a second violation of the Academic Integrity Policy, the Dean of Students or designee will verify the student's prior record.
The Dean of Students or designee will:
- Notify the student of the second violation.
- Provide written notice of dismissal in accordance with the Academic Integrity Policy.
- Explain the student's appeal rights and procedures.
Appeal of Dismissal
A student may appeal the dismissal decision by submitting an appeal to the Academic Committee within the timeframe specified in the dismissal notice. See Student Right to Appeal.
The Academic Committee will review the appeal and issue a final decision.
Enrollment Status During Appeal
Students who timely appeal a dismissal decision may continue attending classes and participating in university activities while the appeal is pending.
Enrollment status will remain unchanged until the Academic Committee issues a final decision.
If the dismissal is upheld, the dismissal will take effect as specified by the Dean of Students or designee.
Record Retention
The Registrar's Office maintains records of academic integrity reports in accordance with institutional record retention requirements.
The Dean of Students Office maintains disciplinary records associated with academic integrity proceedings.
